Terms of Service

Last updated 7 November 2026

ezForms is a free service, from eSpiritz, for making forms, quizzes and surveys and collecting the answers. These Terms are the agreement between you and eSpiritz ("we", "us") about using it. Please read them. By signing in, or by using ezForms in any other way, you agree to them.

1. Who these Terms are for

These Terms apply to everyone who uses ezForms. There are two kinds of people, and some rules differ between them:

  • Form creators: people who sign in with Google to build forms, share them and read the responses.
  • Respondents: people who open a form link to fill one in. A respondent may or may not have to sign in, depending on how the creator set the form up.

If you use ezForms for an organisation (a school, a company, a club), you confirm that you are allowed to accept these Terms for it.

2. Your account

You sign in to ezForms with a Google account. We do not ask for, and never see, your Google password. Google tells us your name, email address and profile picture, and we use them to create and recognise your ezForms account.

  • You must be at least 13 years old, or older if the law where you live requires it, to create an account. If you are younger, a parent, guardian or teacher must set the form up for you.
  • Keep your Google account secure. You are responsible for what happens under your account, including forms created, shared or deleted from it.
  • One account is for one person. Do not share your sign-in with others; use the sharing features (editors) instead.
  • Tell us quickly if you think someone else has used your account.

3. ezForms is free

ezForms has no paid plans, upgrade prompts or locked features. We do not charge you, and we do not show advertising.

To keep the service fast and fair for everyone, the product has reasonable limits (for example on the number and size of questions, answers and attachments, and on how fast requests can be made). We may change these limits, and we may add or remove features. If a change would seriously affect forms you have already published, we will try to give notice in the product first.

4. Your forms and your content

Everything you put into ezForms (questions, descriptions, images, themes, answer keys, and the responses you collect) is "your content". You own it. We do not claim any ownership of it.

You give us a limited licence to host, store, copy, display and process your content, only as needed to run ezForms for you and for the people you share it with. That includes showing your form to respondents, grading quizzes with your answer keys, showing responses to you and your editors, and making backups and technical copies. The licence ends when your content is deleted, except for copies that cannot be removed immediately.

You are responsible for your content and for the way you collect and use the answers. That includes having a lawful reason to collect personal information, telling respondents who is collecting it and why, keeping it safe once you export or download it, and deleting it when you no longer need it. For the responses you collect, you are in control of the data and ezForms only processes it for you (see our Privacy Policy).

5. Acceptable use

Use ezForms lawfully and with respect for other people. You must not use it to:

  • break the law, or help anyone else to;
  • send spam, run phishing or scam forms, or trick people into giving you information, money or access (for example by pretending to be a bank, a company, a school or another person);
  • harass, threaten, defame, discriminate against or exploit anyone, or publish hateful, violent, sexually explicit or illegal material;
  • collect information about children in ways the law does not allow, or sexual or intimate content of anyone;
  • infringe anyone's copyright, trademark, privacy or other rights;
  • upload or link to malware, or try to harm the service or other users;
  • probe, scrape, overload, reverse engineer or bypass the security, limits, timers, spam protection, passwords or access controls of ezForms or of other people's forms;
  • cheat in a quiz or exam run by someone else by using tools, scripts or accounts to get around its rules or timer;
  • sell, resell or rebrand ezForms, or remove our name or logo from the places where they are shown to respondents.

6. Information you must not collect

Forms are easy to share and responses are readable by everyone you give access to, so do not use ezForms to collect:

  • passwords, PINs, security codes or one-time codes of any kind;
  • payment card numbers, bank account details or other financial credentials;
  • government identity numbers (passport, national ID, tax or social security numbers) unless the law requires it and you have secured a proper process elsewhere;
  • health, biometric, genetic, political, religious or similar sensitive information, unless it is truly needed, you have a clear lawful basis and the explicit consent of each person.

If you must collect any personal information, ask only for what you need, and say clearly in the form why you need it.

7. If you fill in a form (respondents)

A form is made and owned by its creator, not by ezForms. When you answer one, your answers go to that creator, who decides what to do with them. ezForms is not responsible for how a creator uses your answers, so check who the form is from before you share anything personal.

Never give passwords, card numbers or other secrets in a form. If a form asks for them, do not answer, and report it to us. You can report a form from its page, or tell us through the contact details at the end of these Terms. We may look at reported forms and remove them.

Some forms need you to sign in with Google so that your email address can be recorded with your response. If the form does this, you will see your account at the top of the form and a way to switch to another account before you submit.

8. Quizzes, timers and results

ezForms can grade quizzes using the answer keys and points set by the form creator, show results to respondents, and limit the time a person has to complete a form.

  • Scores are calculated automatically from the creator's answer keys. The creator is responsible for the keys being right and for any decision made from a score, such as a grade, a pass mark or a selection.
  • A timer runs on our servers, so refreshing, closing or reopening the form does not give extra time. When time runs out, the answers saved so far are submitted automatically, as long as the page is open or is reopened. Answers saved in a draft are not guaranteed to be submitted if the browser stays closed.
  • Timers depend on your connection and device. Do not rely on ezForms alone for high-stakes exams where a lost connection, a late submission or a technical fault could cause serious harm. Creators of such tests should have a back-up plan.
  • Practice mode, preview mode and similar features can show results without saving a response. Nothing in them counts as a real submission.

9. Sharing, editors and links

You decide who can fill in your form (anyone with the link, or only people you add or who are signed in), who can edit it, and who can see the responses and the summary page.

  • Anyone who has your link and meets the form's rules can open it. Links can be forwarded, so treat them like keys.
  • Editors you add can change the form and see its responses. Unless you switch it off in sharing settings, editors can also change sharing and add other editors. Choose them carefully.
  • A password on a form is checked on our servers before the questions are sent. It keeps casual visitors out, but it is not a replacement for proper security for sensitive data. Do not use a password you use anywhere else.
  • If you turn on the public summary, anyone with its link can see the summarised results (not the individual responses).
  • You are responsible for sharing something by mistake. You can change access, unpublish a form or stop accepting responses at any time.

10. Deleting things

When you move a form to the trash it stays there for 30 days so you can restore it. After that it is deleted permanently, together with its questions, settings and responses. You can also delete a form permanently yourself from the trash, and delete individual responses.

Deleted content cannot be recovered. Please export what you need first.

11. Our property

The ezForms name, logo, design, software and the ready-made templates and cover images are ours or are used with permission, and are protected by law. We give you a personal, non-exclusive, non-transferable licence to use ezForms and its templates to make your own forms. This does not give you the right to copy the service itself or to use our name or logo as your own.

If you send us feedback or ideas, we may use them without owing you anything.

12. Other services we rely on

ezForms works with other services, and their own terms apply to them:

  • Google, for signing in (and for the fonts used to display forms).
  • SpacetimeDB Maincloud, which hosts our database and runs our back-end logic.
  • Video and image hosts, when you add a video (for example YouTube) or an image from a link to a form. The people who view the form connect to those services directly.

We are not responsible for services we do not control.

13. Availability and warranty

We work hard to keep ezForms running and your data safe, but ezForms is provided "as is" and "as available", without promises that it will be uninterrupted, error-free or fit for a particular purpose. Features may change. Things may occasionally break or be unavailable, for example during updates or because of problems with our providers.

You should keep your own copy (by exporting responses) of anything you cannot afford to lose.

14. Limits on our responsibility

To the fullest extent the law allows, eSpiritz is not liable for indirect or consequential losses, such as lost profits, lost opportunities, lost data or harm to reputation, or for losses caused by a creator's content or decisions, by a respondent's actions, by third-party services, or by events outside our reasonable control.

Because ezForms is free, our total liability to you for anything arising from using it is limited to the greater of 100 US dollars or the amount you paid us (which is zero). Nothing in these Terms limits liability that cannot be limited by law, such as for fraud or for death or personal injury caused by negligence. If you are a consumer, you keep any rights that the law of your country gives you and that cannot be waived.

You agree to compensate us for losses we suffer because your content or your use of ezForms breaks these Terms or someone's rights, to the extent the law allows.

15. Suspending or ending your use

You can stop using ezForms at any time. You can ask us to delete your account and data (see the Privacy Policy).

We may remove content, limit features, suspend or close an account, or disable a form link if we reasonably believe that these Terms are being broken, that people are at risk, that the law requires it, or after a valid complaint. Where we can, we will tell you why and give you a chance to fix the problem first.

16. Changes to these Terms

We may update these Terms, for example when ezForms gets new features. The "Last updated" date above shows the current version. If a change is important, we will tell you inside ezForms and may ask you to accept the new Terms again. Using ezForms after a change means you accept it.

17. Law and disagreements

These Terms are governed by the laws of the place where eSpiritz is established, without regard to conflict of law rules, unless the consumer laws of your country say otherwise. We would like to solve any problem informally first: please contact us before starting any formal process. Courts in the place where eSpiritz is established can decide disputes that cannot be settled, unless the law gives you the right to go to the courts of your own country.

If a part of these Terms is found to be unenforceable, the rest still applies. If we do not enforce a rule straight away, we do not give up the right to enforce it later. You may not transfer your rights under these Terms to anyone else without our written permission.

18. Contact

Questions about these Terms, a report of abuse or a legal request: contact eSpiritz, the maker of ezForms, using the contact details published with the product, or through the Help button inside ezForms.
